Sreehari,

As mentioned earlier, for your 11c1:0620 chipset modem, do NOT use
Carrier Check = no

Also use you regular login name.

As Antonio mentioned earlier, we need details on the failure.  Before
starting a dial up, do:
# tail -f /var/log/messages

Then open a 2nd console to do the dialout from.
After the failure, send the List the record from the /var/log/messages
