Dialout should not require a
Carrier Check = no

It could be a protocol problem, occurring after the CONNECT has been achieved.

Assuming that your IP (Iinternet provider) is UTStar.com
Instead of your LoginName  use:   UTSstar.com/LoginName
This is a syntax required by IPs which use the CHAP verification protocol.
