You could try moving the sound card to a different pci 
slot.  There could be a hardware timing issue because 
of where it is located. I've experienced this location 
sensitivity in the past.

I really don't have any other advice.  Your system 
isn't working how it is supposed to and I can't see 
why.  It appears you've done everything right.
