i notice that the DocBook Makefile does support an XMLTOFLAGS
variable.  i'm wondering if that can be overridden to get that full
ToC.  sadly, i'm not *that* much of a docbook processing expert.

no, xmlto is just a simple frontend to run the right xsltproc commands
to turn the docbook source into the expected output format ... the
trick lies in the xsl fragment

of course, i have no idea what said trick is :)
