yup, that's *exactly* what i'm seeing for the generated HTML for the
kernel API.  good.  it's reproducible.  :-)

i notice that the DocBook Makefile does support an XMLTOFLAGS
variable.  i'm wondering if that can be overridden to get that full
ToC.  sadly, i'm not *that* much of a docbook processing expert.
