$ sudo ./slmodemd -s -c ITALY --alsa hw:0,0
some of the options are Not appropriate to your
card 10b9:5459

-s is only necessary for 10b9:5451 cards
--alsa is only necessary with ALSA modem drivers, not including slamr
hw:0,0  specifies the audio card, not the modem.
Audio cards can be responsive to the wvdialconf test,
but cannot later manage COMM functions.

For your hardware, the command should be:
$ sudo ./slmodemd -c ITALY /dev/slamr0

Double check your version with
./slmodemd --version
should be 2.9.11
It OK you can copy it to standard position /usr/sbin/slmodemd
