 I think easiest solution for Fred is to first ask the author to
remove your name and immediately submit a new version. If it does not
work, go to the WG chairs. The chairs will certainly get them remove
your name.

I think it would be outrageous to ask every author's approval at the
submission time. The system already informs every author of the
submission and asks for approval. So you know that somebody is using
your name if that is the case.
