To answer myself: Sending code 180 on incoming call does the trick :-)
The server then creates a tone signalling the caller that the remote
side is ringing.
  pjsua_call_answer(nCallID, 180, NULL, NULL);

Does anyone have a clue on the second question? Is there a way to get a
callback whenever a connection is lost, either because it is closed or
because the internet connection or server is down?
