Can't be done. The Cokin adapter is a sort of permanent addition to the front of the lens. It screws into the front of the lens, and then the Cokin filter holder slides over it.

What Andy wants to do is screw his polarizers into the Cokin adapter, but, first, the inside has no threads, and second, it's so thin that one cannot get a polarizer in that diameter, especially since the filter manufacturers only make filters for standard lens sizes.
