well this is OT but I think you all want to know what lead to it (not
technically, but mostly administratively)
first of all i suggest finding the second volume of the biography of Physics
Noble prize winner Richard Feynnman, porobably found in your local library.
this volume has a big part on his participation in the Discovery disaster
research comittee.
what his remarks end up to is:
" it was so hard and costly to meet the specs and safety requirements (in
design and operation), that Nasa decide to lower those to an attainable
level"
exactly what was aired on the maintenance cost cut downs in the columbia
case...
hmm that kind happens when administrators loose all contact to reality...
