It depends on how much "value add" the distribution has around the kernel. The well-known distributions all use an initrd to bring userland up and they also have more or less distribution-specific bootloader management methods. If you want to run your own kernel you have to make sure that it is configured and installed to fit what your distribution expects in terms of bootloader and initrd, unless you want to break free from the distribution's expectations and assumptions and manage more things manually. All of this isn't always so well documented by distributions.
