Edit /etc/wvdial.conf ,perhaps with
# gedit /etc/wvdial.conf
removing the symbols ; < >
and replacing with your personal info
and adding a line
Carrier Check = no

Thereafter, after activating the modem and
# wvdial

Modem drivers occassionally die.  Restart by
Ctrl-C   , to stop wvdial
Ctrl-C   , to stop slmodemd
# modprobe -r snd-intel8x0m
# modprobe snd-intel8x0m
unloads & reloads the driver.

Then activate again and
# wvdial
