I am sorry for not adding the command
# chkconfig slmodemd --add

I forgot it the first time.  Another thing I have overlooked is the following:

You may safely delete the following lines, they are not needed, even
though *they are not part of the problem*

from start procedure below in slmodemd script

As for the following things:

/usr/sbin/slmodemd is the actual slmodemd to be used for dialing it
should not interfere with the script(startup script) slmodemd in
/etc/rc.d/init.d/ folder.  This is strange?  I wonder what Marv
advises here?

For the line 83 unexpected end of file you can probably put a done
command at the very end and check to see if it gives you same error
message?
