Leave the modem unattached during bootup for now
With
$ sudo gedit /etc/default/sl-modem-daemon
change to
DONTSTART=1
as the System seems to be guessing that your need slamr
Shut it down manually this time
$ sudo /etc/init.d/sl-modem-daemon stop

The should be a file /etc/modprobe.d/sl-modem-daemon.modutils
with at least a line:
alias char-major-243 slusb

$ sudo modprobe -r slusb
to unload in case slusb is loaded

$ sudo tail -f /var/log/messages &
to watch kernel messages.
Attach the USB modem
slusb should autoload with dynamic creation of ports. Check with:
$ ls -l  /dev/ttySL0  /dev/pts/N

Try finding the modem with:
$ sudo wvdialconf
If found
$ sudo gedit  /etc/wvdial.conf
entering your personal infor, deleting the symbols ; < >
and adding a line:
Carrier Check  = no

For details:
$ man wvdial.conf

Shut  COMM  channels displayed by
$ ifconfig
with commands like:
$ sudo ifconfig eth0 down
or DNS service will later fail
The "lo" loop back channel is OK

Then try a dialout with:
$ sudo wvdial

For the shutdown problem:
Ctrl-C to stop wvdial
$ sudo modprobe -r slusb
Then shutdown
