Don't worry about the Carrier warning as I get it on a modem which is
functioning (a bug report is needed)

 Best run
# slmodemd -c USA --log --debug=2 --alsa hw:0,6
to produce a log & better isolate the failure point during a sub
sequent dialout.

It your earlier success was on the same hardware (please verify) try
backing down to earlier kernels until a success is recovered.  Linux
is quite tolerant to backing down to earlier kernels.
