If you will install gnu gk, netfilter h323 patch is not needed anymore.
You can find more info at http://www.gnukg.org or at http://sourceforge.net/projects/openh323gk.
A good start would be to search the list archive at sourceforge.net
for proxy topics - lots of configuration examples and explanations for various things.
