I'm using the following setup:
/etc/rcS.d/S09brltty is a symlink created with:
ln -s ../init.d/brltty.sh S09brltty

/etc/init.d/brltty.sh has:
#!/bin/sh
/sbin/brltty

This will start brltty just after S05keympmaps whihc is just after
kernel-boot.
