Perhaps start again with some basics like going to a shell
and using "aplay some.wav" and fiddling with alsamixer to
see, or hear, if you can get something working. You can
then paste any suspicious errors into an email to this
list.

i discovered an interested thing.
after system boot i switched to the console without logging into kde 4.2.
sound works.
after i log to kde the sound stops working. what i mean is that aplay executes without errors (i tried from the console, from a terminal, with root, without root) but no sound is heared.
i also checked the kde mixer, kmix, and all the levels are ok and not muted. i also checked with alsamixer, all ok.
something strange is happening with kde it seems.
