Following up my own message, I now have asfxload working and it appears
that I had failed to load all the required modules: I had missed

snd_emu8000_synth 14820 0
snd_opl3_synth 13636 0
snd_seq_instr 7232 1 snd_opl3_synth
snd_ainstr_fm 2144 1 snd_opl3_synth

In the sources (linux-2.6.18.3/sound/drivers/opl3/, for example) these
modules use the valid names snd-opl3-synth with "-" instead of the more
usual "_". This is why I missed them, and I wonder whether the module
auto loader scripts may have missed them for the same reason...

This has not fully resolved my problem, for using pmidi or aplaymidi or
playmidi only plays a single note and then "hangs" consuming cpu time.
I suppose my next move should be strace or maybe gdb if I can sort out
sources and compile one of the programs. Any suggestions?

With the extra modules manually loaded as above,
/proc/asound/oss/sndstat now looks sensible..
