is your lcd a 12.1 tft ?

no more 640x480, lets try 800x600, add below to your Section "Screen"
    DefaultDepth 16
    SubSection "Display"
            Depth     16
            Modes      "800x600"
    EndSubSection

and below to your Section "Monitor"
    HorizSync    28-40
    VertRefresh  60

the corresponding  mode is 800x600@60 , does it work?
