(WW) R128(0): config file hsync range 31-96kHz not within DDC hsync range 28-65k

(WW) R128(0): config file vrefresh range 55-160Hz not within DDC vrefresh range 
48-65Hz

 According to the monitor's EDID, your Section "Monitor" should have:

        HorizSync   28 - 65 
        VertRefresh 48 - 65

  instead of the MUCH wider range you have now.  The modes the server
was picking based on the range you specified are obviously outside
of the monitor's capability.
