More info (XF86Config / log etc.) would be helpful.
my experience with 320x200 resolution was  a non recognised graphic card in X 3.3.6
This is why it would be helpful to know whether you are running 3.3.6 (which would basically be
reasonable with the -in our days- rather modest setup you seem to have - basically most "modern"
versions of kde * my experience only goes up to 2.2...* work perfectly well with 3.3.6...)
or 4.x.x 
If you have 3.3.6 you should be able to get 16 bits with 1024x768 using the accelerated SVGA
server (XF86_SVGA)
with 4.x.x you would have to use the unaccelerated vesa driver which means lousier performance...
