I once had an odd problem similar to yours in that it seemed 
unexplainably out of place. The reason was that I had configured a 
konqueror profile (which was only slightly different) and had it 
assigned as the default profile for konqueror. With every restart of 
konqueror the old phenomenon (I can't remember now anymore) was back. 
Even if this doesn't sound like your problem, konqueror profiles may be 
the source of it. Make sure you use default profile.
