SquirrelMail checks some variables before saving email to Sent folder and plugins attached to compose hooks can change saving process by modifying those variables. I would try to eliminate ******* and send_confirmation from debugging process first. You don't want to fix issue, you want to understand why it happened so that it won't happen again. If disabling of these plugins does not fix issue, make backup of user preferences (username.pref file or all entries with user=username in database userprefs table) and reset them to default ones (logout from SquirrelMail, delete preferences, login to SquirrelMail).
