You are confusing folder prefix with folder delimiter. In Cyrus
$default_folder_prefix can be set to empty string (''), 'INBOX.' or
'INBOX/'. Period ('.') is used in $optional_delimiter setting, if you use
default Cyrus 'unixhierarchysep' configuration.
Please reapply SquirrelMail cyrus configuration preset.

Check your Cyrus imapd.conf and show 'unixhierarchysep' setting.

That's not default setting value. Your SquirrelMail configuration does not
allow automatic creation of special folders. If cyrus
'autocreateinboxfolders' option is not working, you are contacting wrong
people and you forgot to mention your autocreate patch version. Please
note that folders are case sensitive on cyrus and your SquirrelMail
configuration differs from folder names used in README.autocreate.
