Browser cache is supplying the results not Squid cache.
Some items cannot be cached so are re-fetched whenever needed, those are the repeat MISS.

6. Refresh browser and Squid shows a huge list of memory hits

Depends on how you refreshed. If the browser simply skipped its cache this is expected. If the browser did a forced reload operation Squid should have REFRESH'ed or MISS'ed as well.

HIT's here are very nice, but I'm not sure it was the wanted thing without seeing what request headers the browser sent to Squid.
