It looks like your web server is hosting some or other unhealthy
process.

Do you have any scripting on your sites ? Do you interact with databases
from your sites ?

You may have a php / perl / whatever script that exhausts your system
memory before the kernel eventually kills it.

You can control the max execution time and memory usage of php scripts
with settings in /etc/php.ini

You know that your apache server threads for some reason consume too
much memory... you just need to find out why.
