I had a similar problem. It seems that
/usr/lib/libgphoto2/print-usb-usermap prints out the wrong values for my
camera. I changed the contents of /etc/hotplug/usb.usermap to match what
I saw in /var/log/messages when I connected the camera, and this fixed
the problem.

In my case I saw this in /var/log/messages:
usb.c: USB device 9 (vend/prod 0x40a/0x568)

while in /etc/hotplug/usb.usermap the second and third fields were:

0x040a   0x0566

Changing the 3rd field to 0x0568 fixed it for me.
