Sems to me this is the proxy task. if the proxy wants to be in the signaling and/or rtp path , actions must be taken in the proxy side. In particular for the rtp to be sent to the proxy, when the proxy sends the 200OK , in the sdp it should include its own ip address, thus the rtp will be sent to it.
