Simply there are several software out there that use the earpiece instead of the loudspeaker. How they do? It's a matter of reverse engineering their application a bit to understand exactly how they do, in terms of system call. It should not be so complex imho, Windows Mobile it's a quite easy platform to reverse engineer.
