Patch-O-Matic is a way to add other features to netfilter which are not part of the mainstream distribution. Regard it as a series of patches, each of which adds something different to the standard netfilter, which happen to have been collected all in one place. Whether or not you want to use it depends on whether you want netfilter to do any of the more unusual or exotic things which people have written the extensions for.
