Did you investigations on windows? Devicemanager?
Which IDs are listed there?

network
modems
serial-ports
diag, nema, at-modem,ndis 

xVID:xPID MI0y\z

Do you have  *.inf-files?

If you have usbsnoop, or something similar - could see if IDs are changing 
while plugging in?
