What direction is the packet that gets the error?  Is it OUT from the 
G45 to the hub?

The reconnect is more or less, once a disconnect occurs.  The reason
for the disconnect isn't clear.  The upstream port on a high-speed link
tells when the downstream port has disconnected by monitoring the
signal amplitudes on the data lines; the change in impedance when the
downstream port's high-speed terminations are removed causes them to
change.  This is explained in detail in section 7.1.7.3 of the USB-2.0
specification.
