setkey in vs fwd policies

Sorry if this is the wrong list.

I was surprised to discover the addition of fwd policies to setkey.
I assume that all outgoing packets (both locally generated and forwarded) still pass through the out policy check, yes?

Are incoming packets now checked after the routing decision, against in or fwd policies, according to the routing decision?

Why was the disctinction made?

Where do these checks happen in relation to netfilter hooks?

I'd greatly appreciate answers to any of these, or pointers to the correct mailing list/kernel developer.
