Any help would be much appreicated as I have 2 of these cards in 2 different machines and they both show this problem.

I havent heard about problems with QAM64 yet. Which firmware version are
you running?
The reason for your problem might be a too big cystal frequency deviation.
This can depend on the chip temperature. It would be nice if you could
confirm this. Do things change if you operate the PC with an open case or
put the card into a better ventilated slot?
With some driver support, the chip can handle higher frequency offsets.
But it is significant effort to implement this, i can't do this right now.
