Maybe you would need to try and move the Modem to another PCI slot?
and/or try to follow the IRQ advice on the output of scanModem:

 The PCI slot 01:01.0 of the modem card may be disabled early in  a bootup
process,  but then enabled later. If modem drivers load  but the  modem is
not responsive, read DOCs/Bootup.txt about possible fixes.
 Send dmesg.txt along with ModemData.txt to discuss@xxxxxxxxxxxxx  if help
is needed.

and try the same commands again.  Also try to run ungrab-winmodem
before the efax call and post all the messages from your commands not
just that it does the same:

$ sudo modprobe ungrab-winmodem
$ efax

Copy and paste them and email them to us.
