Congratulations for working it through.

Yours is the first reported success under a Ubuntu 2.6.28,
 kernel 2.6.28-11-generic of the HDA card hosted modem
Codec: Motorola Si3054
Vendor Id: 0x10573055

We may later ask you to run some diagnostics for us.

gnomePPP is just a graphical front end to wvdial.
But it writes its setup configuration file to a hidden file in your
home folder. something like .wvdialconf  After running its seup
Display it with:
$ ls -a ./wv*

You may have to enter manually the:
Carrier Check = no

If you have trouble using it, just back to using the command line wvdial.
You will still have to do the slmodemd setup command.
