This is a step forward (below), that is the test without the
sl-modem-daemon installed.
Please continue that way, as sl-modem-daemon provides no additional
functionality and may complicate issues for modems working through the
snd-hda-intel driver.

Between succcessive tests, please do a
# killall slmodemd
and then start again with the slmodemd command

Get some more detail on the failure by activating with:
# slmodemd -c CANADA --alsa --log --debug=2 hw:0,6

Do the wvdial attempt again in the 2nd console.
Send  us the logfile after the failure.
