Your core problem is:

Try detaching as many external devices as possible before the next Linux boot.
Then rerun scanModem to see if the "disabled" disappears.

If not do:
$ sudo hsfconfig --diagnose
It will write the output file to the /tmp/ folder.
Send it to "Support (Jonathan)" <support@xxxxxxxxxxxx>, who is the
expert on these modems.
