Here above shows that you apparently used a USB modem with device /dev/ttyACM0

Here wlan0 is another interface that is not needed when you are trying
to connect with dialup.
It needs to be shut off to troubleshoot the dialup connection.

$ sudo ifconfig wlan0 down
and all others too like eth0, eth1 etc.

Try one thing at a time.  You are trying to do too much.  If you want
to connect with slmodemd use that and that only.  When attempting to
connect, open a terminal and type
$ sudo tail -f /var/log/messages

send only the parts when the connection is taking place and when it fails.
