scanModem reports that you need agrsm package:

Support type needed or chipset:	agrsm

Use agrsm-20080203a.tar.gz  at

Read Modem/DOCs/Agrsm.txt

However a package with "a" at the end of 3a.tar.gz does not exist, the
closest match is the following:

http://linmodems.technion.ac.il/packages/ltmodem/sv92/agrsm-20080203.tar.gz

 The PCI slot 00:07.0 of the modem card may be disabled early in
 a bootup process,  but then enabled later. If modem drivers load
 but the  modem is not responsive, read DOCs/Bootup.txt about possible fixes.
 Send dmesg.txt along with ModemData.txt to discuss@xxxxxxxxxxxxx
 if help is needed.
