This indicates it is not primarily a driver problem.
Rather there is some other Process competing for resources which is
killing the connection.

Looking at your original ModemData.txt

Uninstall the sl-modem-daemon package, as it is useless for your modem:
$ sudo apt-get --purge remove  sl-modem-daemon

Blacklist modem drivers not needed with:
$ sudo gedit /etc/modprobe.d/blacklist-modem
so final lines are:
blacklist snd-atiixp-modem
blacklist snd-via82xx-modem
blacklist snd-intel8x0m

Please download and used the newest scanModem and send the List the
ModemData.txt
We want to assess what processes are sharing the modems IRQ/interrupt

After a dialout failure, do copy out the terminal section of dmesg
