It is not apparent to me that a clean test has been done.
Please after a boot up do:
$ ls -l  /dev/slamr0  /dev/ttySL0

If these devices are not reported, manually do:
#  modprobe ungrab-winmodemd
#  mknod -m 660 /dev/slamr0 c 242 0 2>/dev/null && chgrp uucp /dev/slamr
This step MUST be done before
$ modprobe slamr

Only then may modem activation proceed by
# slmodemd -c USA  /dev/slamr0
Then there should be a report of port creation:
    /dev/ttySL0  ->  /dev/pts/N  , N some number

Once there has been such a creation, the next functional test is:
# wvdialconf   /etc/wvdial.conf
