Hi Lee, 
Some people have reported success using Hylafax, since
apparently Efax can be very picky config-wise.
IIRC someone did e-mail the list reporting success
faxing with 11c11040 modems.
You can search the archive at linmodems.technion.ac.il

I would otherwise forward you the e-mail, but I am at
a conference, and as such do not have access to my
full e-mail archive.
