My guess is due to the slmodemd already having been run by the bootup scripts,
so that it cannot be reloaded

You could however
$ sudo  /etc/init.d/sl-modem-daemon  stop
and then
$ sudo   slmodemd -c USA /dev/slamr0
Note that "sudo slmodemd"  is essential, that is Admin/root capacity is needed.

The IRQ 16 does not seem to be shared with other hardware.

and further, the slamr driver is at least accessing the modem firmware
because  "SmartLink Soft Modem"  is written in the firmware.

6) Your /etc/wvdial.conf  is OK

So it is not yet transparent why there is the later failure with:

Note that everytime you run
$ sudo  wvdialconf  /etc/wvdial.conf
the prior wvdial.conf is overwritten.  So check that it is OK before
your next testing.

With respect to shutdown,  just do
$ sudo  /etc/init.d/sl-modem-daemon  stop

Or if you started slmodemd manually, then in that console you can just:
CTRL-C
Then
$ sudo modprobe -r slamr
to unload the driver
$ sudo modprobe slamr
will reload, after which you could manually do:
$ sudo   slmodemd -c USA  /dev/slamr0
and in a 2nd console:
$ sudo wvdial

Try and get somemore information by:
$ sudo   slmodemd -c USA  --log --debug=1 /dev/slamr0
which will write a log file you can later send us after the dialout attempt,

To compile your own software under 2.6.22-14-generic., you need to
install the packages:
linux-headers-2.6.22-14  and linux-headers-2.6.22-14-generic.
Let us know if you have an alternate ethernet CONNECT and we will instruct.
