Please do send your complete ModemData.txt  to the List, so we have a
complete body of information.

There is some possibility that your snd-hda-intel will have to be
upgrade to a newer ALSA version, to achieve support.  See
http://linmodems.technion.ac.il/bigarch/archive-seventh/msg02144.html

But we need the complete ModemData.txt to guide you.
