For your 201f:545a SmartLink-based modem, you'll need the slamr driver. 
Precompiled drivers are available for your kerne, get them from 

http://linmodems.technion.ac.il/packages/smartlink/Ubuntu/slamr-2.6.20-15-386.tar.gz

Extract under Ubuntu with

	$ tar xzvf slamr-2.6.20-15-386.tar.gz

browse to the extracted directory :

	$ cd slamr-2.6.20-15-386

And run the installation script :

	$ sudo ./setup

Edit the slmodemd configuration file :

	$ gksudo gedit /etc/defaults/sl-modem-daemon

Finally, restart slmodemd :

	$ sudo /etc/init.d/sl-moem-daemon restart

You can then run the wvdialconf utility, which should detect your modem :

	$ sudo wvdialconf /etc/wvdial.conf
