Chas,
Since we do not know what you have got in your machine [because your ModemData.txt is truncated and comes from an obsolete version of scanModem; it only shows the PCI Id of your modem and your kernel, but not which software you should and might or not have installed] nor what you have done before trying to configure wvdial and then trying to configure it (there is one way, no more), it is essentially impossible for me to help you.
