Yes likely an ISA card
Also read the instructions from SoftModem.txt  for getting modem
chipset iinformation under a Microsoft boot.
No way we can help until you acquire some chipset info.

On the possibility that it is a Controller Chipset, check if the modem
is found by:
$ sudo wvdialconf  /etc/wvdial.conf
