It's the audio test in checkbox that appears to trigger issue. It's a python problem and the source code is here: http://launchpad.net/checkbox/0.7/0.7.1/+download/checkbox_0.7.1.tar.gz I tried searching for "hotkey" but it wasn't that easy. :) Perhaps the audio test is calling an external tool which is then triggering the issue.
